Home is best: ED iCARE

Embracing a ‘home is best’ philosophy, ED iCare aims to avoid unnecessary hospital admissions for complex-care patients aged 70 or older who come to ED.

Rolled out at Mount Saint Joseph (MSJ) Hospital in April 2016, ED iCare has now made the move to St. Paul’s Hospital Emergency Department in February 2017.

Embracing a ‘home is best’ philosophy, ED iCare aims to avoid unnecessary hospital admissions for complex-care patients aged 70 or older who come to ED and, instead, appropriately align them with community resources for their continued care. It’s a patient-centred, personalized, approach that uses a multidisciplinary approach led by a geriatric emergency nurse, social worker and transition service coordinator.

On Day One at St. Paul’s, 22 seniors presented to the ED with only four requiring admission. The ED iCARE team facilitated six discharges of patients who were identified as moderate to high risk for readmission.
